Case Management

 

The Case Management Team is responsible for coordinating and assuring the continuity of all services and supports for consumers of Community Partners including accessing services provided by other agencies. Presently 204 individuals receive Case Management services.

Each individual's services vary according to his or her needs. These needs are identified annually for each individual in a service plan developed by a team. This team includes the individual, Case Manager, guardian, other family members, friends and appropriate staff from Community Partners and/or other agencies. 

Case Managers are responsible for monitoring the service plan and the quality of the provided programming. Because supports are individually determined, the supports for each person are unique. Case Managers assist in accessing community resources to enhance the individual's quality of life.
