DEVELOPMENTAL SERVICES POSITIONS

EARLY INTERVENTION/ESS INTAKE COORDINATOR: Seeking an Intake Coordinator to join our Early Intervention Team! This person will be responsible for the intake process for all referrals for children birth to age 3 to Family Centered Early Supports and Services (FSESS) Early Intervention in accordance with regulations and best practice, provide service coordination and conduct developmental evaluations with children birth to three. Qualifications: two years experience with infants and toddlers with disabilities and their families required. Shall meet requirements of He-M 510.05(b) of the state FCESS Standards and RSA 171-A. MS, MSW preferred. Full-time position, flexibility required.

SUPPORTIVE BEHAVIORAL LIVING SKILLS COACH: Assist clients with daily living skills while promoting skills necessary for increased independence. Implement behavior supports and plans and individual service plan. Training provided. High School diploma or equivalent required. Experience or degree in the Human Services preferred. Must be physically able to assist consumer in and out of vehicles and to push wheelchair. Schedule is normally Monday – Friday but may vary.
 

DAILY LIVING SKILLS INSTRUCTOR: Description: Assists consumers of Residential Services to develop daily living skills while facilitating and promoting the skills necessary for increased independence. Works directly with the consumers in their homes and in the community, to provide a quality daily living environment in accordance with NH State certification standards. Qualifications: High school diploma or equivalency required. One year residential service experience preferred.

DAY VOCATIONAL DIRECT SERVICE STAFF: Description: Work directly with consumers in their daily activities and prepare and participate in assigned consumers’ ISPs. Act as liaison between consumers and community members. Must become medication certified according to HeM 1201s. Qualifications: High School diploma or equivalent required. Experience in the Human Service industry preferred. Must be able to assist consumers in and out of vehicles.

VOCATIONAL SPECIALIST: Individual will be responsible for the assessment and coordination of career planning activities and vocational placement for clients with developmental disabilities and/or mental health issues. Includes coordination of services with treatment teams and the Department of Vocational Rehabilitation. Facilitates job matches and evaluates job accommodations as necessary. Maintains employer relations and provides employer supports. Bachelor’s degree in human service field required. Ability to assess placement, provide support to consumers, and provide clear communication with prospective employers required. Some flexibility of schedule required.

RESIDENTIAL PROGRAM COORDINATOR: Responsible for implementing and monitoring quality residential services programs as assigned. Responsible for assuring compliance with NH state certification standards at all times. Responsible for training and supervision of direct care staff. Monitors training of Home Providers. Monitors day to day fiscal expenses for assigned programs. Liaison between direct service staff and management team. Bachelor’s degree in related field plus three years experience in a human services field with at least one year of supervisory experience. Must have excellent verbal and written communication skills. Must have some financial skills. Must be flexible depending on needs of the organization.
 

 

BEHAVIORAL HEALTH SERVICES POSITIONS

 

RESIDENTIAL SERVICES COORDINATOR: Description: Manager of a multidisciplinary community-based treatment team providing mental health services to consumers who have intensive support needs. Coordinate functional and habilitative services as needed to promote successful community integration and tenure, avoid or decrease hospitalizations, promote independence, and assist with the restoration and obtainment of skills necessary to move toward a less restrictive level of care. Qualifications: Master’s Degree in Social Work, Psychology or other related field required. A minimum of 3 years experience with the mentally ill population required. Two years of supervisory experience preferred; one year required. Ability to plan and perform diversified duties requiring extensive knowledge of mental health treatment and recovery-based initiatives.

INDEPENDENT LIVING SPECIALIST: Description: The Independent Living Specialist provides community based residential support services to improve client success with independent living skills in their chosen least restrictive residential setting. Provides individual and group support, advocacy, assessment and rehabilitation services to CSP clients referred to the Assertive Community Living Team. Provides medication education and administration. Performs preliminary crisis assessment and links clients to crisis resources, as clinically indicated. Qualifications: Bachelor’s Degree in Psychology, Social Work or related field preferred. Equivalent combination of education and experience considered. One year related experience preferred. Must be able to exercise independent judgment and use discretion in the performance of these duties while operating within the limits of standard practice.

YOUTH AND FAMILY THERAPIST: Description: Responsible for providing therapy for children, adolescents and families in a community mental health center setting, using a family centered approach to treatment. Position offers supportive multidisciplinary team atmosphere and weekly supervision. Qualifications: Master’s Degree in Social Work, Counseling, Marriage & Family Therapy or related field required. Licensed or license eligible in New Hampshire preferred. Experience with children and families required.

CLINICAL CASE MANAGER: Description: Clinical Case Manager, Assertive Community Living Team: Experienced, Master’s level clinician to join a dynamic, multidisciplinary team providing intensive, wrap-around supports to adults with severe and persistent mental illness. Will coordinate all aspects of care with consumers, families, guardians, outside agencies and other systems. Provision of direct individual/group therapy and supports, crisis assessment and intervention, advocacy and resource linkage. Qualifications: Experience with recovery principles, IMR, dual disorders, and/or the legal interface a plus. Master’s degree in Social Work, Psychology or related field with a minimum of two years experience preferred.

ARNP: Description: Full Time Psychiatric Nurse Practitioner needed to work with our psychiatric team in meeting the behavioral health needs of people in our region. As a member of our team, this position will be responsible for developing and managing a collaborative care model addressing the mental health and physical health of people in partnership with our local Community Health Clinic. Qualifications: Master’s degree in Social Work, Counseling, Marriage & Family Therapy or related field. Current NH ARNP License, NH RN License, current DEA required. Must maintain ANCC certification as Clinical Nurse Specialist. One year experience as Nurse Practitioner in a mental health care setting preferred.

CASE MANAGER - CSP: Description: Responsible for development, implementation and monitoring service plans, providing mental illness management services in the community. Works as part of the CSP team assisting people with mental illness in acquiring and maintaining skills and resources necessary for management of their mental illness while working toward recovery. Qualifications: Bachelor’s Degree in Psychology, Social Work or related field required. Valid driver’s license, dependable transportation and auto insurance required.
