Partners in Health

Help for Children with

Chronic Health Conditions and

Support for their Families

To receive Partners In Health services a family must live in Strafford County
And have a child age birth to 21 with a chronic health condition such as:

Allergies
Asthma
Aids
Cancer
Cystic Fibrosis
Diabetes
Muscular Dystrophy
Spina Bifida
or any Ongoing Illness or Physical Disability

We help families by:

• Identifying needs
• Connecting families to resources & services
• Providing respite, social & recreational opportunities
• Collaborating with schools, health care providers and other community agencies.

The program is administered by Community Partners and the The Hood Center for Children and Families, a program of The Children's Hospital at Dartmouth and The Dartmouth-Hitchcock Medical Center in Lebanon, NH.
