Partners in Health

Help for Children with

Chronic Health Conditions and

Support for their Families

 

To receive Partners In Health services a family must live in Strafford County
And have a child age birth to 21 with a chronic health condition such as:

Allergies
Asthma
Aids
Cancer
Cystic Fibrosis
Diabetes
Muscular Dystrophy
Spina Bifida
or any Ongoing Illness or Physical Disability
