High School Transition

for Students with Disabilities

2005 Transition Manual - free download

The transition years, usually thought of as between ages 16 to 21, are a period of time that bring about many changes. These changes are felt by both the student with a disability as well as his or her family. Minors become adults. Services such as SSI and Medicaid become available that may not have been available as a child. Seeking guardianship may be necessary to ensure needs are met. And plans definitely need to be made for life after high school.

New programs are sought. Services are no longer an entitlement as was schooling, and families learn of the potential that services may not be immediately available after graduation, This may result in the placement  of the consumer's name on a waiting list for services such as case management, residential, and/or day/vocational programming.

The Transition Coordinator, a member of the Family Support team, will assist you through this period of great change and will stay involved until adult services are secured. The Transition Coordinator has developed a manual to assist students and families called Transition: The High School Years. To view it online you must have Acrobat Reader , a free download, installed on your computer. Also Visit our Links Page for Transition related websites to visit.
